Please fill out the following information to request an Alternative Work Arrangement (AWA).
Once submitted, this information will be sent to your AVP/Dean and VP for review and approval. If approved, it will be reviewed by HR for compliance purposes. You will receive notification of approval or denial from HR. If approved, you will be prompted to complete your AWA agreement digitally. You may contact HR for more information and the next steps if denied.
