Students can use this form to submit an appeal of academic dismissal and/or an appeal of satisfactory academic progress to reinstate financial aid eligibility. You will be given the option to indicate which type of appeal you are submitting at the start of the form. Students submitting both types of appeal will only need to complete the form once.
