The following forms are for use by faculty and staff only in regards to academic information. These forms, provided by the Office of the Registrar, pertain to advising, registration, student records, graduation and academic data.

Please note: You will need to use your login username and ID number to access some forms. We're in the process of moving our requests to single sign-on authentication. You will log in using your St. Edward's credentials for the surveys. If you have already logged in to myHilltop, you will not need to log in again. Some of the forms have small user lists, and you will need to contact the Office of the Registrar to be granted access.
